BY FIVE’s innovative partnership is helping more families in regional Victoria to access specialist paediatric care locally and has been recognised by the Victorian Early Years Awards 2020.
By Five’s Wimmera Southern Malle Specialist Paediatric Support Partnership is helping to embed specialist expertise within the local primary care services that families trust and use every day.
Many children living in remote and regional areas are referred to specialist services for health and development issues that could be resolved locally. The Specialist Paediatric Support Partnership’s innovative shared care initiative is successfully connecting specialist expertise with local primary health providers via digital health and improving the confidence and capacity of primary health services to solve complex care issues in partnership with families.
The partnership is a collaboration between Wimmera Southern Malle Maternal Child Health providers and the Royal Children’s Hospital. It extends to over 50 health, education and family service providers working with families with children, as well as the Department of Education and Training, Department of Health and Human Services and the Murdoch Children’s Research Institute.
By Five’s Wimmera Southern Malle Specialist Paediatric Support Partnership was awarded ‘Creating Collaborative Community Partnerships’ at the Victorian Early Years Awards 2020.
This partnership is significantly improving health and wellbeing outcomes for children by connecting families with quality and timely care in their own communities.
