The gestational age at birth in Australia has slowly but steadily declined over the past 30 years, mainly due to increase in planned births (caesarian sections, inductions of labor). The effects of this decline in gestational age to child health are evident at many levels – intensive care, paediatric care, community, and school.

Illness and separation at a critical time for infant-parent attachment have adverse effects on infant and parent mental health. COCOON, which stands for the Circle of Care Optimising Outcomes for Newborns, is the patient-centred, family-integrated and baby-led model of care we designed to minimise these adverse effects in the complex medical and surgical NICU population, although the core principles can be applied in any healthcare setting.

Hospitalisation for treatment of serious illness can place infants, and their families, at risk with respect to their mental health. Trauma responses are common, and optimal infant-parent relationship development may be disrupted. Significant additional environmental and psychosocial burdens were placed on this group in 2020 as a consequence of measures adopted to protect the community from COVID-19.

Respiratory syncytial virus (RSV) is a leading cause of severe acute respiratory infections (SARI) in children in countries throughout the world. The Royal Children’s Hospital (RCH) emergency department, wards and intensive care unit are full of patients with RSV infections (including bronchiolitis) from April to September, with a peak in June-July. The Melbourne Children’s campus has a long history of RSV associated research, including clinical trials encompassing preventative, supportive and therapeutic strategies.

Recent cases of conflict around medical treatment for seriously ill infants and young children have raised a number of questions about the nature, consequences and ethics of disagreement in neonatal intensive care.

Helping tiny babies to breathe after birth – short- and long-term consequences

Before the advent of modern neonatal intensive care in the 1970s, many tiny babies died soon after birth, most commonly because they lacked pulmonary surfactant, leading to respiratory failure.  Professor Lex Doyle first treated the breathing problems of tiny babies in the mid-1970s during his basic training in paediatrics.

Probiotic Use in Preterm Infants

Probiotics are defined as live microorganisms, which when administered in adequate amounts, confer health benefits on the host. Their exact mechanism of action is currently unclear, however, their beneficial effects are likely to result from a variety of actions including, alteration of the innate commensal microbiota, modification of gut barrier function, and modulation of intestinal immune responses.
