2015 Consumer Forum: The transitioning patient

Consumer_Forum_NewsThe 2015 RCH Consumer Forum will take place on Friday 6 March.

Whether you are an RCH consumer representative, are interested in becoming one, or are simply an interested parent, carer or young person, there will be sessions at the forum that will be of interest to you.

This year’s forum will focus on communication and information sharing in the context of the transitioning patient.

Transition is the planned move of a patient’s care from a paediatric to an adult health care provider. It usually occurs between 16 and 19 years of age and involves careful preparation of the patient and their family.

Speakers at the forum will include: Professor Christine Kilpatrick, RCH CEO; Evelyn Culnane, RCH Transition Manager; Dianne Tucker, RCH Director of Quality Systems; Shane Thomas, Family Advisory Committee Chair; and Derek McCormack, Senior Manager, Knowledge Exchange, Parenting Research Centre.
