{"id":4779,"date":"2014-12-24T02:35:52","date_gmt":"2014-12-23T15:35:52","guid":{"rendered":"https:\/\/blogs.rch.org.au\/news\/?p=4779"},"modified":"2015-04-29T10:58:18","modified_gmt":"2015-04-29T00:58:18","slug":"good-to-go-at-the-rch","status":"publish","type":"post","link":"https:\/\/blogs.rch.org.au\/news\/good-to-go-at-the-rch\/","title":{"rendered":"Good to Go at the RCH"},"content":{"rendered":"<p><strong><a href=\"https:\/\/blogs.rch.org.au\/news\/files\/2014\/12\/QOC_Patient-Matthew.jpg\" data-rel=\"lightbox-image-0\" data-rl_title=\"\" data-rl_caption=\"\" title=\"\"><img loading=\"lazy\" decoding=\"async\" class=\"alignright wp-image-4781 size-medium\" src=\"https:\/\/blogs.rch.org.au\/news\/files\/2014\/12\/QOC_Patient-Matthew-280x280.jpg\" alt=\"QOC_Patient-Matthew\" width=\"280\" height=\"280\" srcset=\"https:\/\/blogs.rch.org.au\/news\/files\/2014\/12\/QOC_Patient-Matthew-280x280.jpg 280w, https:\/\/blogs.rch.org.au\/news\/files\/2014\/12\/QOC_Patient-Matthew-300x300.jpg 300w, https:\/\/blogs.rch.org.au\/news\/files\/2014\/12\/QOC_Patient-Matthew.jpg 504w\" sizes=\"auto, (max-width: 280px) 100vw, 280px\" \/><\/a>The purpose of the RCH\u2019s Good to Go (G2G) program is to ensure efficient use of beds and improve patient access within existing resources. <\/strong><\/p>\n<p>This is measured by the average multiday length of stay with a goal to reduce this by 10 percent, thus creating opportunities for more patients to have access to inpatient beds<b>.<\/b><\/p>\n<p>\u201cGood to Go was conceived to ensure the patients can go home when they are clinically ready to go home,\u201d Deputy CEO John Stanway said.<\/p>\n<p>G2G focuses on continually improving bed management and discharge processes, through standardising practices and identifying more efficient ways of caring for patients, to improve bed availability.<\/p>\n<p>\u201cEveryone has to operate at their optimal level and operate together, otherwise the hospital won\u2019t perform as well as it should,\u201d John explained. \u201cOur clinicians can only be as good as the support they get, they won\u2019t be able to achieve excellence if patient rooms aren\u2019t cleaned in time, or finance holds up approval for medical supplies.\u201d<\/p>\n<p>G2G is made up of multiple initiatives to improve access, care and patient flow across the hospital; including electronic patient journey boards, long stay patient rounds and criteria led discharge.<\/p>\n<p>Criteria-led discharge helps avoid unnecessary delays in patient discharge, as the patient doesn\u2019t have to wait to see their consultant before they can go home.<\/p>\n<p>\u201cCriteria led discharge creates a shared understanding between carer groups, the patient and their family of the clinical criteria that need to be met for the patient to go home,\u201d John said. \u201cAll groups then work together to send the patient home as soon as this criteria is met.\u201d<\/p>\n<p>Outcomes to date:<br \/>\n\u2022 Multiday length of stay (LOS) target of 4.4 days has been achieved and exceeded<br \/>\n\u2022 The hospital is consistently discharging 20-25 patients prior to midday (compared with 6-8 patients prior to G2G implementation)<br \/>\n\u2022 Hospital Initiated Postponements have remained below target since Nov 2012<br \/>\n\u2022 Admission to wards from ED within four hours has increased from 22 percent to 55 percent in April 2014<br \/>\n\u2022 The number of long stay patients and their collective bed days had decreased dramatically.<\/p>\n<p><strong>Complex Care Pilot Program\u00a0<\/strong><\/p>\n<p>The RCH has developed a complex care pilot program to support our most complex patients and their families.<\/p>\n<p>\u201cThis program seeks to transition the model of care for complex patients from reactive management to optimising a more planned approach to maintenance of wellness and minimise hospitalisation as a result,\u201d Deputy CEO John Stanway said.<\/p>\n<p>The pilot program will offer advanced strategies to better support the care and treatment of children with complex healthcare needs and is built around three key components.<\/p>\n<p>\u201cThe program will provide care at inpatient, outpatient and community levels and would \u2018dovetail\u2019 with existing services, both within RCH and across community service providers.
